2. Select I am with a provider office
3. Enter your e-mail address.
4. Enter a subject of “PHA Medicaid Provider Portal Access Request”
5. Select the topic of “I have an issue with a CIM account or need a new account”
6. Complete the form and press Submit.
7. An Ayin representative will review your request and respond via email.
